Neurological Conditions

1 Health Conditions
a. Co-morbids associated with disease process to support diagnosis
b. Pyelonephritis
c. Septicemia
d. Decubitus ulcers, multiple stage 3 and 4 wounds

2 Palliative Performance Scale (PPS)
a. Less than or equal to 40%
b. Mainly in bed
c. Needs assistance with all ADLs

3 Inability to maintain hydration and caloric intake
a. Difficulty swallowing
b. Aspiration pneumonia
c. Weight loss of > 10% during the last 6 months
d. Weight loss of > 7.5% during the last 3 months
e. Serum albumin less than or equal to 2.5 gm/dl

4 Coma (supporting terminal diagnosis)
a. Abnormal brain stem response
b. Absent verbal response
c. Absent withdrawal response to pain
d. Serum creatinine > 1.5mg/dl

5 Respiratory
a. Dyspnea
b. Continuous supplemental oxygen
